Key Position Details: Workforce Dynamics Hybrid Workforce: The Access Center supports a hybrid workforce. As a hybrid employee you will be given the opportunity to telecommute from an approved remote workspace. You will also be able to balance your work and personal life more effectively as you will have more control over your schedule and work arrangements. However, your home base will remain Allina Access Center offices at the Minneapolis Commons building. Thus, the following provision will remain part of Allina's Remote Work Agreement and Department specific procedures. Access employee across all shifts meeting department and organization telecommute requirements are approved to work remotely as a hybrid employee. You may be required to work in-office for your shift in order to support the orientation of new team members. This is part of the Allina Remote Work Agreement. Supervisors will provide advance notice via email of this requirement. It is the responsibility of the employee to provide notification of any equipment/technology failure impacting the ability to perform assigned duties following Allina's Remote Work Agreement and department-specific procedures. In the event you are unable to perform your duties as assigned, it is the expectation that the employee report to the Access Center offices in Minneapolis to complete their shift within 1.5 hours of the known downtime or equipment failures. Schedule: Each employee is assigned a specific rotation for weekends and holidays. The schedule is posted two weekends in advance. Employees are encouraged to submit their requests for days off and plan their Paid Time Off (PTO) for vacations well in advance. Shift Start Times 7a, 3p, 11p (8hr) 0.8 FTE (64 hours per two-week pay period) 8-hour evening/night shifts Every other weekend rotation **Applicants must live within commuting distance of the Allina Commons. Job Description: Manages, coordinates, and facilitates patient placement from external referring sites and internal departments. Coordinates pre-planning and predictive census monitoring. Facilitates collaboration between referring and receiving departments and providers. Principle Responsibilities Facilitates patient placement for admission and transfer patients from outside facilities and internal departments. Assigns patient to appropriate bed based on guidelines. Pre-assigns expected surgical patients. Collaborates with Charge Nurse and others to facilitate placement and discharge in a timely manner. Obtains patient information from referral sources. Facilitates phone requests from referring sites and communication between physicians as needed. Conducts pre-pend, appointment, approval, and other processes in Excellian electronic medical record. Manages process with follow-up calls after pre-determined timeframe. Monitors Emergency Department track boards for potential admits. Communicates to facilitate transfers between units. May facilitate physician and/or service paging. May page out Stroke Code notifications. May page Cath Lab team for Interventional cases. May track team check-in and follow up. May page or contact specialists on call. May facilitate/schedule for Cardiovascular Interventional cases. May coordinate physician assignment lists for newly assigned patient rotation. May page the daily census to stakeholders. May page surge-alert messages. Conducts follow-up communication with referring sites. Provides transfer and admission information. Obtains necessary information and reports from referring facility. Completes documentation in electronic record and department databases. Tracks patient assignment, arrival, and flow. Adds information from referral sources to medical record. Tracks statistical and quality data. May facilitate protocol / procedure pertaining to patient death notifications and documentation. May document billing-related information. Utilizes predictive parameters to project census changes. Tracks patient assignment, arrival, and flow. Coordinates information relevant to scheduled and unscheduled admissions, discharges and transfers to provide status of bed availability. Monitors inpatient, outpatient, and procedural unit census for bed availability. Monitors procedure schedules for forecasting. Other duties as assigned. Required Qualifications Associate's or Vocational degree 2 to 5 years in healthcare field, including medical terminology 2 to 5 years in computer skills Preferred Qualifications Bachelor's degree 2 to 5 years in Epic / Excellian electronic medical record Physical Demands Sedentary: Lifting weight up to 10 lbs. occasionally, negligible weight frequently Pay Range Pay Range: $23.22 to $31.83 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
